Understanding Medical Practice Overhead
Practice overhead includes the operating costs required to provide services and run the business. Major categories may include provider compensation, staff payroll and benefits, rent, insurance, EHR systems, billing, supplies, equipment, technology, compliance, and professional services.
Operating margin is the percentage of revenue remaining after the entered operating expenses are subtracted. A positive operating margin does not necessarily equal cash available to owners because taxes, debt payments, capital purchases, distributions, and other items may also affect cash flow.
Expense ratios show how much of the practice's entered revenue is used by each category. They are useful for tracking trends over time, but this calculator does not establish an appropriate industry benchmark for your practice.
Common Medical Practice Overhead Mistakes
- Comparing billed charges with expenses instead of comparing net collections with expenses
- Ignoring payroll taxes, benefits, malpractice insurance, and support-staff costs
- Failing to separate provider compensation from other staffing costs
- Overlooking EHR, billing, IT, equipment, compliance, and recurring subscription costs
- Treating operating profit as cash available for owner distributions
- Ignoring debt payments, taxes, capital purchases, and working-capital needs
- Assuming expense ratios are automatically comparable across specialties or markets
- Treating a public calculator as a compensation, staffing, valuation, or compliance analysis
- Entering patient, claim, payer, or protected health information into a public tool

Schedule a Medical Practice Financial ReviewEducational use only. This calculator provides a simplified, general estimate of healthcare-practice overhead, expense ratios, operating margin, and operating profit or loss based solely on aggregate financial assumptions entered by the user. It is not medical, healthcare, tax, legal, accounting, financial, valuation, reimbursement, billing, coding, compliance, HIPAA, compensation, staffing, or investment advice. It does not determine fair market value, reasonable compensation, staffing requirements, reimbursement, payer contract performance, billing accuracy, coding compliance, tax liability, practice value, or any compliance obligation.
Actual practice overhead, collections, expenses, margins, cash flow, compensation, and profitability may differ materially because of payer mix, reimbursement, billing, coding, claims, denials, staffing, benefits, provider compensation, facility costs, equipment, technology, market conditions, taxes, debt, capital expenditures, regulatory requirements, and other factors. Consult qualified healthcare, legal, tax, accounting, financial, billing, coding, compliance, payroll, compensation, and other professionals before making staffing, compensation, pricing, operational, investment, or business decisions.
