How Medical Practice Break-Even Works

Break-even is the approximate level of revenue or visit volume needed for a practice to cover its fixed and variable costs. A practice may need more visits, stronger net collections, lower overhead, or a combination of these changes to reach profitability.

Net collection per visit is not the same as a billed charge. It represents the average amount the practice expects to collect after contractual adjustments, payer reimbursement, write-offs, and collection experience.

Fixed costs generally remain relatively stable regardless of patient volume, such as rent, base staff payroll, software, insurance, and provider base compensation. Variable costs generally increase as patient volume or revenue rises.

Common Medical Practice Financial Mistakes

  • Using billed charges instead of net collections
  • Ignoring denials, write-offs, bad debt, and delayed collections
  • Underestimating staff payroll, payroll taxes, benefits, and provider compensation
  • Ignoring malpractice insurance, EHR costs, billing costs, licenses, and compliance expenses
  • Failing to separate fixed costs from variable costs
  • Assuming a full schedule always equals profitability
  • Ignoring payer mix and collection performance
  • Using patient-level information in a public financial tool
  • Treating this estimate as a practice valuation or compliance analysis

Schedule a Medical Practice Financial Review

Educational use only. This calculator provides a simplified, general estimate of potential medical or healthcare practice revenue, costs, break-even volume, operating margin, and operating profit based solely on aggregate financial assumptions entered by the user. It is not medical, healthcare, tax, legal, accounting, financial, valuation, reimbursement, billing, coding, compliance, HIPAA, Stark Law, Anti-Kickback Statute, employment, or investment advice. It does not calculate payer reimbursement, Medicare or Medicaid payment, procedure-code reimbursement, claims, patient charges, collections, clinical outcomes, provider productivity requirements, compensation compliance, practice valuation, or any tax liability.

Actual practice performance may differ materially because of payer mix, contracted reimbursement, billing and coding, claims denials, patient collections, staffing, provider compensation, overhead, regulations, licensing, compliance obligations, malpractice costs, market conditions, practice operations, tax rules, and other factors. Consult qualified healthcare, legal, billing, coding, compliance, valuation, tax, and financial professionals before making business, clinical, employment, compensation, investment, or operational decisions.

Do not enter patient names, dates of birth, medical record numbers, diagnosis codes, procedure codes, insurance member IDs, claim information, treatment information, appointment information, or any protected health information.
